Injury Reporting Protocol
PRMLA INJURY REPORTING PROTOCAL
TEAM TRAINER:
In any case where there is a concussion or serious injury, i.e. a player is unable to return to a game/practice vs. missing a shift or two and then returning (ex. just winded), the OLA Participant Serious Injury Reporting (PSIR) Form must be filled out by the team Trainer. This shall be done before the player leaves the arena to ensure that the incident is documented with the cause of the potential injury, along with any initial symptoms that exist with the player. If an injury is not identified immediately during an OLA/PRMLA sanctioned event, the OLA PSIR Form should be filled out as soon as possible by the team Trainer. If in doubt, the OLA PSIR Form shall be completed to ensure the incident is documented. The team Trainer will make the determination as to whether or not a player is to cease participation in all OLA/PRMLA sanctioned activities, until they receive a medical examination, and they are not to be influenced in that decision by Coaches or parents of the player; caution is always the best response.
The OLA PSIR Form must be filled out by the team Trainer and a copy must be emailed to the following people: the Head Coach, the parent or guardian, and the Risk Management Director (raiderslacrosseriskmanagement@outlook.com). The Risk Management Director will then inform the President.
Important: If you have, or suspect you have, a player with a concussion or serious injury, do not allow that player to participate in any further OLA/PRMLA sanctioned activities until AFTER the player has received a professional medical evaluation and has provided a note to the team Trainer, from the medical professional, indicating that it is safe for the player to resume participation.
These protocols are applicable during all OLA/PRMLA sanctioned games, practices, and activities.
Concussions:
The OLA has adopted the 5R Policy as its basis for management of concussions in all OLA sanctioned events (including games, practices, tryouts, scrimmages, and any other event where the risk of concussion may exist):
a. To RECOGNIZE all concussive/suspected concussive injuries;
b. To REMOVE from play for proper evaluation any participant suspected of having a concussive injury;
c. To REST any participant who is confirmed by a licensed medical professional as having suffered a concussive injury;
d. To REQUIRE any participant who is confirmed by a licensed medical professional as having suffered a concussive injury to follow a Graduated Return to Play Plan;
e. To RETURN the participant to play when both the participant (and, if applicable, the parent/guardian) and the licensed medical professional have determined it is safe to do so.

PARENTS/GUARDIANS:
If your child has sustained a concussion or serious injury as a result of their participation in a RPMLA sanctioned event, please follow the steps below:
- Ensure that you receive a copy of the OLA PSIR Form, that is filled out by the team Trainer and ensure its accuracy.
- Ensure that your player receives a medical examination by a trained medical professional.
- Ensure that your player follows the appropriate protocols for their recovery.
- Ensure that your player obtains a note from a Medical Professional indicating that it is safe for your player to return to OLA/PRMLA sanctioned activities and provide this note to the team Trainer.
